The second FaithAction ID drive and the second issuance in Forsyth County will be held on Friday, April 8, 2016 at Southeast Plaza, 3065 Waughtown St., Winston-Salem.
Doors will open at 9 am. Applicants must attend a required orientation session at 10 am or at 1 pm. Applicants must provide:
Photo ID (one of the following):
- Passport.
- ID from country of origin.
- Drivers License (expired or valid).
- Matrícula Consular.
Current address proof (one of the following)
- Utilities bill (power, water, phone)
- Bank statement.
- Current lease agreement.
- Medical record.
Price per ID is $10 (cash only).
